1. Make your Booking via the 'Book my appointment' button

2. Check your emails for your security Code

3. Return here to complete the Questionnaire.

Please complete our Questionnaire, the Questionnaire provides me with a comprehensive understanding of your Health and Wellbeing.

When you make your booking:

If you select Telehealth, your appointment will be conducted via your computer or phone from the comfort of your home.
If you select a Face to Face Consultation, I will see you in my rooms on the day of your appointment. I am fully vaccinated.
